Does living in a small municipality change any of these rules?

No. Every requirement described here comes from U.S. federal law and is applied the same way regardless of which Canadian town the applicant writes on the form. Geography affects travel plans and which port is convenient; it does not affect eligibility.

An employer keeps saying it will sponsor the applicant. Is that a route?

Not yet. Sponsorship is a word people use for several unrelated things, and it settles nothing until someone names the classification, identifies who files what, and says who pays. Ask for those three answers in writing before rearranging a life around the offer.

Can a tidy binder make a weak case strong?

It cannot. Organisation makes the facts legible, which is worth a great deal, but an officer weighs the facts and not the binder. Where an element is missing, the honest move is to say so and work out who could supply it.
